Sample custom form?

The Forms Designer function looks very promising, but it's a bit intimidating the first time.

Does anyone know of a sample form (ideally a recreation of the default form)?

I think it would be easier for me to tweak an example form than to start from scratch the first time.

has anyone actually designed a form?

I can provide an email address if people don't want to make their form public. I'm not looking for anything relevant to a particular business, just for some example to get me started.

In addition to the tutorials, Maximizer provides samples here:

Is there any way to get the default form for customization? the samples have some fields and columns missing, and seem to make it so that a contact made using them will always show with that form instead of the default form..

I'm pretty sure not.

I notice that the samples were buggy and had trouble doing simple things properly. As I recall, the obvious was the one with a user-defined field that is a list of items: Normally you can just check the box, but the sample had a Rube Goldberg setup with a picklist and 2 text boxes interacting with each other (and not really working right anyway).

I think we all just want to tweak the default form, but I consider the custom form capability pretty worthless.

I think that's the intended behavior and arguably makes some sense. But you can unset "use alternate form" on the contact.

I cant seem to find the use alternate form checkbox anywhere.. where is this setting?
also is there a way to have it automatically switch use alternate form off when a contact is created using custom form? the only reason i want to use custom form is so a user-defined field can be seen and chosen from first page..

"use alternate form" is a user-defined field

I didn't find this, but perhaps someone else did.

If it's not intrinsic to Maximizer, you can try doing a global edit at the end of the day (try hitting the spacebar when prompted for the new value).

Join the club!
Given that the example for user-defined fields doesn't work, I gave up hope.

In Maximizer 8, 9, and 9.5 you can confgigure which UDFs will show on the first screen by configuring list of 'Key Fields' in the Maximizer Administrator.

In the Maximizer Administrator goto File->Preferences and then the Key Fields Tab.

You can Add lists of up to 8 fields to different areas of Maximizer (the Address book, Opportunities, etc) and these lists will show up on the first screen when inserting and modifying records.

i see that, but it doesnt seem to show them if using a custom form, also cant seem to get it to default to not use the custom form when viewing the entries(i want to use the custom form so it is more intuitive for users to fill in a user defined field(i.e it gives them a box for them to fill in the value for the user defined field(would be better to have a checkbox but couldnt figure out how to configure that..
