I am creating a custom form using several user defined fields and I have run into a couple of problems.

Date Field: I want to create an optional date field (preferably a datepicker field) which can be left blank until the user decides to enter a date. Currently, every time I create a date field it defaults to the current date.

Yes/No Fields: How do I create these?

OK Button: I've succesfully added a "Cancel" button to my form and it works fine, but my "OK" button is grayed out. Any ideas?
