Aaah! What am I doing wrong with Reportsmith?

I am a newbee with Maximizer. I have been greatly humbled in my attempts at generating a Custom Report for prospects. The first part of the report is coming up with Company names but no first and last names for contacts. And then it is coming up with Company names, with contact names. But many are the same company as the top of the list.
All I want is a report that has the company name, the contact name, and some user defined fields. What am I doing wrong? I have read the book, but I am obviously missing something. Is it how I have my address list set up? Why would it be duplicating companies? I await humbly for guidance.
