Double company name

I am trying to print labels or envelopes but when I have a contact included in the label/envelope it will print the company name twice.

If I have just a company (no contact) it will print the company name once. If I go to the address and click on "do not include company name" it will leave the name off all together.

How do I make it stop? Slapping the computer does not work I already tried that.
