Phantom file (icon) in Library

We have a phantom file in our Company Library. It is a file that was deleted but the icon in the library remains. Right clicking on the file icon and choosing 'delete' generates a message that basically says the file has already been deleted. How do we remove this nuisanace icon?

Have you tried doing

File > Address Book Verification

in the Administrator module?

Yes I have. It really is quite strange I can move the file, rename it, view file properties - just can't get rid of the blighter.
